Physicians, attorneys, case managers and family members may contact M & M Home Care about a referral. Tell us which services are needed and where the patient will receive care.

  1. Call 248.599.2410. Discuss the injury, location and requested services.
  2. Review the intake requirements. The office will explain the orders, records and other information needed.
  3. Confirm the next step. Availability and the care or evaluation arrangements are reviewed individually.

Please call before sending medical records or other sensitive information by email. Ask the office how to provide the required documents.
